FINDING US & PARKING

We are conveniently located at Two Main Street in Stoneham. To ensure your visit is completely smooth and stress-free, here is what you can expect when you arrive:

  • Our Building: Look for the two-story brick professional building marked “TWO” right above the main entrance. The front entrance is fully handicap accessible, and an elevator is available to easily bring you up to our office.

  • Parking: There is a large private lot with ample parking at the front and rear of the building, making it easy to park and step right inside.

Once you step through the doors, our front desk team will be right there to warmly welcome you and get you comfortably settled.

FORMS & PREPARATION

To ensure your appointment goes as smoothly as possible, please review the important reminders below and take care of your paperwork in advance.

  • Arrival Time: For new patients, please plan to arrive 15 minutes before your appointment time to finalize paperwork and get comfortably settled.

  • Pre-Appointment Medications: If your dentist or medical doctor has prescribed any pre-appointment medications (such as premedication antibiotics), please ensure you take them exactly as directed prior to your arrival.

  • New Patient Forms: Prefer to complete your paperwork ahead of time? Download and print our forms below, or call our office and we will gladly mail a packet to your home.

We provide three different in-house payment plans for our patients without dental insurance, as well as 0% interest patient financing options. Check out our plans below:

GeNeral Plan - Adult1

  • Two examinations (Includes medical history update, cavity, gum, and oral cancer screenings, and an oral hygiene review)
  • Two Dental Cleanings

Periodontal Plan - Adult1

  • Two examinations (Includes medical history update, cavity, gum, and oral cancer screenings, and an oral hygiene review)
  • Four Periodontal Cleanings per year (Excludes deep scaling and root planing)
  • Four Fluoride Treatments as Needed
  • Four Digital Check-up X-rays
  • 10% reduction in fee for Dental Services only

General Plan - child1

1. Terms & Conditions of our Membership Plans: Available exclusively to patients with no dental insurance. This is not an insurance plan and is exclusive to Stoneham Family & Cosmetic Dentistry. Membership coverage term is 12 months. All fees for additional services are due at the time of treatment. The 10% fee reduction applies strictly to necessary, non-elective clinical dental services performed in our office. This savings does not apply to retail products (including Sonicare), clear aligner therapies, or elective cosmetic treatments such as Botox and dermal fillers.

We welcome patients with a wide variety of insurance plans. For plans where we are an independent (non-contracted) provider, we are equipped to handle paperwork and claim processing for you.
